Guide for extracting menus for editing, and reinserting edited menus

I see questions every now and again asking how to extract menus for editing, or how to get edited menus back into a DVD. I can think of 4 scenarios where you might want to extract, edit, and replace a menu (or 2 or several), although there are more, I'm sure.
